1. Interacting with professors and students in person is:
extremely important to me
not necessary to me
somewhat important to me


2. Classroom discussion and group activities are:
somewhat valuable but not necessary to my learning process
very necessary as these activities always help me understand the material better
rarely helpful to my learning process


3. When it comes to studying I:
am very disciplined and productive studying by myself
need to interact and share with other students while studying
fairly disciplined however I sometimes get distracted


4. I consider myself:
somewhat organized depending on the situation
well organized
disorganized


5. I see myself as someone who is:
very motivated and disciplined, I always finish anything that I start
fairly motivated, but sometimes I need reminders to finish things
unmotivated, I have difficulty starting things and rarely finish them on time


6. I find that I complete tasks:
always ahead of schedule
at last minute if at all
if I am reminded to


7. I retain material best when:
I hear it, I understand material better when delivered verbally
I see it, I understand material best when I read it
I don't have a preference and can learn by listening and viewing


8. The urgency for me to take this course right now is:
very high, I need it for a specific goal
not entirely urgent, I can take it on campus later
very low, can be postponed


9. I find computers, e-mail and other technologies:
frustrating and difficult to use
a little challenging but I am fairly comfortable using them
easy to use


10. My access to VCRs, TVs, computers and email:
is unlimited, these technologies are always readily available to me
limited, I don't have one or more of these technologies
somewhat limited, I can get access without much difficulty


11. My reading and comprehension skills are:
slower than average
above average, I rarely have difficulty understanding text
about average, occasionally I require assistance in understanding text


12. When given an assignment, I usually:
work through the instructions independently
work through the instructions , then ask for help
need instructions explained by my instructor first


13. After handing in an assignment I need my grade and instructor feedback:
within the next few days, otherwise I forget the subject
right away or within a day, otherwise I get frustrated
within a few weeks so I can review what I did


14. When assessing and monitoring my own progress I:
am able to keep an eye on my progress without frequent feedback from my instructor
have difficulty and require constant feedback from my instructor
appreciate receiving frequent feedback from my instructor, but it is not necessary


15. Attending a regular schedule of classes on campus is:
very easy for me, I am very flexible with few commitments
somewhat easy, I would have to make some adjustments to other priorities
very difficult, I have work, family and personal commitments during typical class times
